Please visit our Health Review and Assessment Clinic if you have been advised by the surgery to complete a health review or assessment.
If you would like to request an appointment for any of our clinics and services, please make an appointment in the following ways:

- By using our online services, Patient Access (If you are not registered to use this service, please visit Patient Access)

Apart from the usual medical services we provide the following nursing services:
- Diabetic reviews
- Asthma reviews (patients can use our Asthma Review form)
- Heart Disease review
- Blood Pressure Checks, including 24 hours BP (patients can use our Blood Pressure Review form)
- Injections and immunisations – adults and children
- Ear syringing
- Weight loss monitoring
- Wound care
- Removal of sutures
- Health promotion / advice
- Smear tests and contraception reviews
- Health checks for patients with severe mental illness or learning disability
Contraception and Sexual Health
Contraceptive advice and advice regarding sexual health issues are from the doctor. Coil and Implants are available from Dr Hipps.
Patients can use our online Contraceptive Pill Review form.
Repeat prescriptions for contraception and screens for sexually transmitted infections are available from the nurse.
HIV tests are available to all patients, please book an appointment with the nurse by submitting our Request an appointment with a Nurse or Healthcare Assistant form if you feel you would benefit from this or any other sexual health screening test.
Travel Immunisations, Including Yellow Fever
If you are travelling abroad, you are likely to need vaccinations against diseases that are not prevalent in the UK. Our practice nurse will give you advice on health issues in the country and area you are going to and will advise you on which vaccinations you require.
Please fill in our Travel Risk Assessment form 6 weeks prior to your departure and before your appointment.
Baby Clinic: New Baby 6 Week Check / Immunisations
When we receive notification of your delivery we will book your baby for a 6 week check by appointment with the doctor in Baby Clinic. We will also arrange an appointment for their first immunisations with the nurse in Baby Clinic.
Available by appointment at 6 weeks post-delivery, linked to your baby’s 6 week check.
Well Baby Clinic
This is a health visitor led clinic for general advice about feeding and development. The clinic is run in the Garden Suite on the ground floor of Gracefield Gardens.
At other times the Health Start Team are available by calling 020 3049 4002.

Seasonal Flu Vaccination Clinic
The flu vaccination is available every year, free on the NHS, to help protect people at risk of flu and its complications.
